This area of service may cover:

Ensuring safe mobility within the community and pedestrian safety

Assisting with accessing and utilizing local community resources

Promoting awareness and education on community safety

Building informal support networks and connections

Enhancing interpersonal communication abilities

Planning leisure, recreational, and social activities

Developing skills to fulfill transportation needs

Health, Safety, and Wellness
This category may involve:

Working alongside individuals to coordinate their healthcare needs, such as physical, mental, or chemical care, as well as arranging activities, social services, and appointments.

Providing training or assistance to help individuals perform self-care tasks, including activities of daily living (ADLs).

Offering health services support as outlined in Minn. Stat. §245D.05.

Assisting individuals in developing resilience through programs like whole health action management.

Helping individuals create and achieve personalized strategies for their health, safety, and wellness objectives.

This service may cover:

Providing reminders, supervision, or training to help with daily household upkeep

Teaching skills to ensure household safety and security

Offering support and advocacy related to tenancy issues

Assisting with financial management, including budgeting, and provided guidance on cooking, meal planning, and nutrition

Promoting healthy lifestyle habits and practices

Help with household chores, including minor maintenance tasks

Note: Individuals are responsible for the cost of replacing maintenance items or products.

This category may involve:

Developing skills for crisis prevention

Supporting sensory and motor development to acquire functional skills
